You can pre-register to save time on your first visit to our clinics.  Simply click here and print the form.  Then fill it out, sign it, attach a copy of your insurance card front and back, a copy of your identification, and send it using our secure fax to 301-657-0031 or 949-437-2080.  This will allow us to contact your insurance company in advance and save you time on your first visit.
